The FBI missed at least five chances to find two of the 9/11 hijackers before their attack. The Justice Department Inspector General has found plenty of foul-ups at the FBI that could have prevented the 9/11 attacks. The IG calls it a "significant failure" by the FBI.
Nearly four years after the attacks, no one in the federal government has been fired for failures leading to the attacks. (WashPost)
